Gordon’s Functional Health Patterns: A Nursing Guide

Gordons Functional health Pattern

In 1987, Marjory Gordon developed the 11 set of Gordon’s Functional Health Patterns to be used in the Nursing Process by nurses to ensure that a more comprehensive nursing assessment is done of the patients.

In each pattern, nurses ask questions to obtain more details that perhaps the patient may not have provided during the initial assessment and/or issues that may have developed during the course of nursing care.

The health patterns include;

Table of Contents

1. Health Perception-Health Management Pattern

This pattern describes how a patient views and understands their health and how they take measures to manage their health. It takes note of healthy and unhealthy practices.

By asking questions related to this pattern, the nurse can identify factors that promote patients’ health such as health-seeking behaviors, and those that are detrimental to their health such as smoking, drinking, and substance abuse.

2. Nutrition and Metabolic Pattern

This pattern describes the food and fluid intake relative to metabolic needs. Nurses can use this pattern to understand the patients’ nutrient supply, fluid balance, the tissue integrity, and immunity.

3. Elimination Health Pattern

Elimination refers to how the waste products are removed from the body through excretion mechanisms of the bowel, bladder, and skin. This pattern elaborates the excretory function of the body and can be used to assess excretion abnormalities such as urinary incontinence, urinary retention, polyuria, dysuria, constipation, diarrhea, and diaphoresis.

4. Activity-Exercise Pattern

This pattern will help the nurse to understand if the patients can perform daily activities such as hygiene, walking, and eating without a problem. A nurse will also be able to understand the systems involved in the activities such as cardiovascular, respiratory, and musculoskeletal.

5. Cognitive–Perceptual Pattern

This pattern assesses the patients’ neurological functions and comprehension abilities such as orientation to person, time, and place, and ability to follow commands.

6. Sleep-Rest Health Pattern

Sleep and rest are vital when it comes to health. However, some patients may have abnormal sleeping patterns such as hypersomnia or insomnia. The sleep-rest pattern enables a nurse to detect these abnormalities.

7. Self-Perception – Self-Concept

The nurse assesses the patients’ attitudes to self, self-identity, and self-worth. It rotates on how patients view themselves by asking the patient questions about their body image, comfrey and self-esteem, and feeling.

8. Role Relationship Health Pattern

This pattern explores the patients’ ability to know their role in the world. Are they satisfied with their role? And are they functional or dysfunctional? Do they have personalities that hinder their relationships and engagements with others?

9. Sexuality Reproductive

The questions asked here by the nurse are to determine the patients’ sexual satisfaction and/or dissatisfaction. Are they satisfied with their sexual orientation? And what is their sex appetite?

10. Coping-Stress Tolerance

To cope is to deal effectively with a difficult situation. This pattern attempts to describe how the patient deals with stressful situations and the effectiveness of his or her strategies.

11. Value Belief Pattern

Values and beliefs are ideologies that are highly esteemed by a person. For patients, values and beliefs can affect their choices or decisions.

Functional Health Patterns

Marjory Gordon (1931- 2015)

Introduction

  • Clinical nurses employ the Functional Health Patterns (FHP) model, created by Marjory Gordon, as a thorough and organized approach to evaluating eleven categories of human functioning. 1
  • The self-administered Functional Health Pattern Assessment Screening Tool (FHPAST) is a questionnaire designed to assess health, functional, and risk issues. 2
  • The Eleven FHP (Health Perception-Health Maintenance, Nutrition-Metabolic, Elimination, Activity-Exercise, Sleep-Rest, Coping-Stress, Self-Concept-Self-Perception, Cognitive-Perceptual, Role-Relationship, Sexuality-Reproductive, and Value-Belief) are interrelated, iterative, and interdependent. 3
  • Functional health patterns is an integral part of nursing documentation. 4
  • Functional Health Patterns are used as an assessment framework to guide nurses in collecting relevant data to drive accuracy in nursing diagnosis. 5
  • By using functional patterns, nurses can conduct a planned and methodical nursing assessment. 6
  • NANDA-I Nursing Diagnosis classification is based on Gordon's functional health patterns.
  • Gorden's functional health pattern includes 11 categories which is a systematic and standardized approach to data collection.7

List of Functional Health Patterns 7

  • Health Perception – Health Management Pattern
  •  describes client’s perceived pattern of health and well-being and how health is managed.

  • describes the pattern of food and fluid consumption relative to metabolic needs and pattern indicators of local nutrient supply
  • Elimination Pattern
  •  describes the pattern of excretory function (bowel, bladder)
  •  Activity – Exercise Pattern
  • describes patterns of exercise, activity, leisure, and recreation.
  • Cognitive – Perceptual Pattern
  • describes sensory, perceptual, and cognitive pattern
  • Sleep – Rest Pattern
  • describes patterns of sleep, rest, and relaxation.

  • Sexuality – Reproductive Pattern
  • describes the client’s pattern of satisfaction and dissatisfaction with sexuality pattern, describes reproductive patterns.
  • Coping – Stress Tolerance Pattern
  • describes general coping patterns and the effectiveness of the pattern in terms of stress tolerance.
  • Value – Belief Pattern
  • describes the pattern of values and beliefs, including spiritual and /or goals that guide choices or decisions.

Major Research on FHP Models

  • Gordon's FHP model application in Heart Failure patient nursing care was linked to a markedly higher quality of life and a lower rate of readmission to the hospital. 8
  • Functional Health Pattern Assessment Screening Tool (FHPAST) can be used to address health risks, overall well-being, and protective actions, as well as a framework for evaluating frailty in elderly hospital patients. 1
  • FHP model facilitate to identify FHPs (client strengths) and dysfunctional patterns (nursing diagnoses). 9
  • A study on relationship of perceived functional health patterns and recovery in people with schizophrenia spectrum disorders shows that functional health is strongly associated with recovery and useful in promoting recovery. 10
  • The application of nursing assessment, which makes use of the patient assessment form based in Gordon's functional health patterns, can enhance students' learning as they develop their nursing processes. 11

Much has been written about the educational life and works of Harriet Merrill Johnson (1867-1934) involving her work as Director of the very first laboratory nursery school in the United States and her revolutionary theories about nursery education. Little to nothing has been on paper about her visiting nursing work for the Henry Street Settlement, Hartley House settlement and other institutions, her unionist work for the Women's Trade Union League, and her landmark work with the Public Education Association of the City of New York introducing visiting teachers and Binet testing in public school education. In 1916, she was one of three founders of the Bureau of Educational Experiments, the later Bank Street College of Education, renowned for its progressive teachers and educators. Already throughout the first two decades of the twentieth century, Harriet M. Johnson together with her lifelong companion Harriet Forbes formed an out-of-the-closet lesbian couple that was non-guardedly open and straightforward about their gender leaning. For several years the Forbes-Johnson duo formed a household together with another pair of women (~ the Marot-Pratt couple). Yet, they in all probability were the first candidly lesbian couple ever to adopt a child, ‗bequeathing' her with both their family names, ‗baptizing' her with a combined hyphenated family name (Forbes-Johnson), and raising her in their ‗two-mother' household. While this case study extensively describes the life of Harriet M. Johnson during the period 1900-1920, the authors certainly did not forget to examine her 1920-1934 educational theorizing about nursery education that until today without any question has been underreported in the literature — even by Bank Street College of Education. We cautiously remind readers that this is a descriptive article. Again, we sincerely hope that we encourage young researchers to build their analysis on this text!

Abstract Generalized anxiety disorder (GAD) is a highly prevalent disorder that has a number of overlapping symptoms with and is frequently comorbid with other affective disorders. Despite its prevalence, the evidence-based therapies (EBTs) for GAD have lagged behind the EBTs for the other affective disorders. The newly developed transdiagnostic psychotherapies may be well suited to the highly overlapping and comorbid presentations of GAD. The present case study involves a veteran with GAD receiving Transdiagnostic Behavior Therapy (TBT), a transdiagnostic behavioral intervention designed for veterans with affective disorders, but has yet to be investigated in GAD, a disorder with a significant cognitive component. The veteran case completed 8 sessions of TBT. Significant symptom improvements were evidenced at post-treatment assessment and sustained at follow-up. The present case study provides very preliminary evidence to suggest that transdiagnostic EBTs, such as TBT, may be a potentially effective treatment for GAD.

Purpose: There is little empirical evidence regarding the nature and components of OS. So, the purpose of this research is to identify the impact of Organizational Silence (OS) on Job Attitudes (JA) at the pharmaceutical industry in Egypt. Design/methodology/approach: Five dimensions of OS are constructed and measured in order to examine their effects on JA at the pharmaceutical industry in Egypt. Out of the 356 questionnaires that were distributed to employees, 315 usable questionnaires were returned, a response rate of 88%. Multiple Regression Analysis (MRA) was used to confirm the research hypotheses. Findings: Results indicate that supervisors' attitudes to silence, top management attitudes to silence and communication opportunities are associated and predict employee silence behaviour. The research has found that there is a significant relationship between OS and JA. Also, the research has found that OS directly affects JA at the pharmaceutical industry in Egypt. Practical implications: This research pointed to the need for organizations to adopt a culture which encourages and urges employees to speak in the labor issues and the non-silence in order for the administration to be able to realize these issues and try to solve them first hand in order to prevent their aggravation. Originality/value: There is little empirical evidence in the literature aimed at defining, analyzing, and coping with silence. It has an impact on the ability of organizations to detect errors. Therefore, this research aims to measure the effect of OS on JA. Based on the findings of this research, some important implications are discussed.

Children having anxiety when separated from the parents are common. But when this anxiety hampers the day to day life then this could be a sign of Separation Anxiety Disorder. This case provides knowledge about the diagnosis and treatment of 5 years old Indian school boy " Rajat Sharma ". After 2 months of regular sessions, Rajat was able to attend school without any problem. The sessions also included family therapy as the parents' relation also seemed to be major cause of child's anxious behavior. The treatment strategy for present case includes behavioral interventions, psychotherapy including cognitive behavioral interventions and family interventions.

The objective of the research is to identify the relationship between Workplace Bullying (WB) and Workplace Anxiety (WA) at Menoufia University hospitals in Egypt. The research community is composed of all employees at Menoufia University hospitals in Egypt. Due to the time and cost constraints, the researcher adopted the sampling method to collect the necessary data for the study. The appropriate statistical methods were used to analyze the data and test the hypotheses. The research has reached a number of results; the most important of which are: (1) the health sector in Egypt is regarded as a sector in which non-negligible amounts of emotional abuse events are seen. Hospitals in Egypt are busy and stressful work environments. They suffer difficult working conditions, e.g. night duties, impossibilities, low wages of employees in public institutions, emotional harassment (Bullying) during academic career and promotion. (Bullying) has a negative impact on business life, (2) WB needs to be explored in a sustained and systematic way because organizations have a responsibility to protect their employees from the psychological harassment of a workplace bully, (3) the degree of WB experience was higher in nurses working in poorer environments, and the nursing work environment turned out to be a significant influencing factor on WB. So, this organizations created an opportunity for violence and could promote such violence, which was significantly affected by the work environment and the organizational culture, (4) the availability of some administrative factors that cause WA such as the lack of welcome of the senior management of the organization to the idea of Telework, and the failure to recruit employees in the degree of career commensurate with their experience and skills, and the existence of competition between the administrative departments of the organization in a manner that causes mutual problems, senior management's criticism of the workers, (5) there is a statistically significant correlation between WB and WA at Menoufia university hospitals in Egypt. The study presents a number of recommendations; the most important of which are: (1) managers should be aware of the concept, types, reasons, methods, consequences and remedies of bullying for their organizations, (2) reducing bullying through improving the workers' awareness about it, (3) there is a need for the development of a structural model that can explain the influential factors and the consequences of WB, (4) conduct positive training courses and focus on the need to provide senior management support to staff members in a manner that prevents WB. Taking into account the appointment of nurses in the appropriate degree of expertise, skills and abilities, (5) there is a need for the development of a reliable and valid instrument that can measure bullying, and that is sensitive to the characteristics of the nursing occupation. Therefore, I suggest an organizational policy development and intervention research to reduce WB, (6) the degree of WA varies in terms of their nature and degree of impact on employees. It may be an impetus for development and improvement, an opportunity for challenge and self-validation, and may be a source of innovation and the emergence of creative ideas, (7) adopting modern administrative methods in work, such as Telework, which ensures the possibility of continuous communication of employees with their organization regardless of their social conditions in different forms, (8) rehabilitation training courses that support the change of individual factors that cause resistance to change.

Background: Childbearing woman needs to start the folic acid intake at least four weeks before conception and continue it throughout the first trimester of pregnancy to help with the prevention of neural tube defects (NTDs). Hence, counseling and awareness by health care professionals influence strongly in order to overcome these burdensome issues to the women and their families. Objectives: To determine the impact of counseling on childbearing age Women for Prevention of Neural Tube Defects by Periconceptional Folic Acid Supplementation in Pakistan. Methodology: A descriptive survey design was carried out to conduct this study in the Obstetrical Outpatient departments of local teaching Hospitals of Lahore. A total of 300 pregnant women were studied to see the impact of counseling of healthcare professionals regarding supplementation of folic acid in order to minimize the Neural Tube Defects. Nonprobability convenient sampling technique was used for the purpose. Data were collected by a self-administered questionnaire and was analyzed by SPSS version 20. Frequencies and percentages were calculated. Results: The results signified that a major fraction of pregnant women 101 (33.6%) used to visit the healthcare professionals after elapsing the critical phase of organogenesis. Although the main source of provision of health-related information was found the health care professionals, it was also observed that only a few of pregnant women used to visit them. Furthermore, health care professionals used to provide counseling for folic acid use in the first trimester of pregnancy, but regarding supplementation of folic acid before pregnancy, no evident authentic information was provided to women of childbearing age. Conclusion: Findings of the study suggested that the health care professionals need to provide awareness at all the levels of health care regarding periconceptional folic acid supplementation. In addition, women should visit the professionals in the early antenatal period as well as before conception. CONTRIBUTION OF PAPER  Birth defects related to folic acid deficiency are as preventable as polio and as a public health measure; equally urgent. Women not currently taking a multivitamin could be motivated to do so if advised by the health care professionals like nurses who provide them holistic care during their stay in hospital as well as community midwives, lady health workers can impart their role.  In Pakistan, due to convenient access, women (primigravida and multigravida both) most likely prefer to take health care advice (After missing their first menstrual period) from community midwives, lady health care workers first before consulting a doctor or gynecologist. Therefore, counseling at primary health care level regarding folic acid supplementation before and during pregnancy should be optimized to reduce the burden of neural tube defects in Pakistan and to

From the moment students are accepted into post-secondary institutions, the focus is on their studies and successful completion. Far less is known about what institutions understand about students' personal stresses and issues with their finances, housing and especially student homelessness. How are these issues identified and managed within post-secondary institutions? What is the role of the post-secondary institution around ensuring students have what they need personally to be successful in their studies? A case study design using an integrated mixed-methods approach was chosen to examine the perceptions of students, administrators, faculty and service providers at one post-secondary institution, regarding experiences with and assistance for students who are vulnerable to becoming, or are homeless. Also explored were existing and needed institutional information and supports for students, and clearly identifying responsibilities of institution and students. Perceptions varied regarding how known and accessible student information and services were for housing, food or finances, and who was responsible to ensure that these students' personal needs were identified and met. Perspectives were similar around the need to improve policies and communications between students and institution staff and services on identifying students at risk, making centralized information and campus services more confidential and accessible, and resourcing service providers to be better prepared and able to assist vulnerable students with financial, housing and other challenges. Institutional policies and procedures need to be more transparent and student-centred, but students also need to take responsibility for their own welfare and seek out confidential campus supports before they experience crises.
